Role of Chase Amputation in Refractory Suppuration of the Index Finger

Mehdi Abakka, Hamza El Ouagari and Mouhssine El Mhadder

Chase’s intervention extends to the treatment objectives by indexing the middle finger which will ensure the pollicidigital grip; maintain strength and respect the aesthetic appearance of the hand. The Chase amputation is an amputation at the base of the second metacarpal, which allows indexing of the middle finger. It has the advantage of ensuring maximum opening of the middle thumb commissure. Indexing the middle finger leads to better integration and usefulness of the finger.

Covid−19 and Simultaneous Thrombosis of Two Coronary Arteries

Raphael Rossi Ferreira

Since the first documented outbreak of a novel severe acute respiratory syndrome inducing Coronavirus in China at the end of 2019, the virus has spread to all continents. Multiple authors have reported an increased rate of thrombo-embolic events in affected patients. 31-year-old male, without any cardiovascular risk factor admitted to hospital with an elevation of Troponin I after seven days of coronavirus diagnosis. Cardiac magnet resonance imaging revealed the presence two infarcts and coronary angiography showed thrombus in right coronary and left anterior descending arteries. We present a serious acute cardiac event in the context of a paucisymptomatic Coronavirus infection.

Management of Uterocutaneous Fistula: A Stepwise Approach

Mini Mathew, Joylene D’Almeida, Ashish Shivendra Singhal, Sujaya V Rao, Prema D’Cunha and Shannon Francesca Fernandes

A fistula is an abnormal tract that connects two epithelial surfaces and occurs as a result of trauma, surgery, or infections. Most uterine fistulae are between the uterus and the bladder (utero-vesical) or between the uterus and the bowel (utero-colonic) due to postoperative injuries or infectious conditions. Utero-cutaneous fistula (communication between the uterus and skin) is a rare condition, accounting for only a few reports in the existing literature. We describe a patient with fistula following a caesarean section who did not respond to medical or surgical (secondary suturing) treatment. She underwent fistulectomy with repair of the uterus and abdominal wall. The procedure led to a successful outcome.

Tigecycline−Induced Triad of Adverse Effects − Acute Pancreatitis, Drug Reaction with Eosinophilia and Systemic Symptoms (DRESS) and Thrombocytopenia: A Rare Case Report

Khalil Zahalka and Tsvi Gregory Avishai

Reports about Tigecycline-induced acute pancreatitis are raising, Tigecycline-induced thrombocytopenia and hypersensitivity reactions are also fairly reported leading to severe clinical complications. We report a rare case of a triad adverse effects associated with Tigecycline treatment in a 5-year-old boy who received Tigecycline for treatment of disseminated mycobacterium abscessus within three months. Tigecycline was a leading cause for all three adverse effects especially acute pancreatitis based on the Naranjo adverse drug reaction probability scale, adverse effects were addressed by a multidisciplinary team and even discontinuation of Tigecycline was done twice with a desensitization re-challenge protocol. Clinicians should be aware of these possible adverse effects especially acute pancreatitis and mainly in high-risk patients.
