The Ultimate Stage of Rasmussen ’ s Encephalitis

Rasmussen encephalitis (RE) is a rare and progressive disease of childhood. We report the case of 14-year-old girl who presented with recurrent partial and generalized epileptic seizures with mental impairment and progressive left sided hemiplegia. EEG showed slowing of the background activity and paroxysmal discharges of slow waves in the right hemisphere. Serial MRI brain scans revealed an important atrophy of the right hemisphere with dilatation of the ipsilateral lateral ventricle. The residual phase, characterized by decrease in seizure frequency and stabilization in the hemiparesis, constitute the ultimate stage of RE.

Malathion Poisoning Presented as Intermediate Syndrome and Organophosphate Induced Delayed Polyneuropathy in Succession: A Case Report

Organophosphate compounds after exposure cause acute and sub acute manifestations depending on the type and severity of the agents like Acute Cholinergic Manifestations, Intermediate Syndrome and Delayed Central Nervous System Complications. It occurs in association with the ingestion of large amounts of organophosphate after the stimulation of cholinergic receptor. We reported a 36 years-old female patient who developed severe Organophosphates Poisoning complicated by Intermediate Syndrome and Organophosphate Induced Delayed Polyneuropathy in India. It is reported to increase awareness of health care workers to this rare complication.

Combined DNA applianceTM and InvisalignTM Therapy without Interproximal Reduction: A Preliminary Case Series

Objective: To use a biomimetic appliance prior to the use of removable clear aligners to reduce the necessity for interproximal reduction in the correction of malocclusions. Subjects: Three adult females aged 23-38yrs. Subject A had a narrow upper arch and mandibular retrognathia; Subject B had crowding and an increased overjet; Subject C had anterior and posterior crossbite. Intervention: All subjects were given optional treatment plans of DNA appliance therapy followed by Invisalign to improve the occlusion. Results: Subject A showed improved mandibular positioning, with improved facial soft tissues. Subject B showed improved posterior occlusion with an increase in upper and lower arch width, resulting in improved facial soft tissues and smile esthetics. Subject C showed improved posterior occlusion, correction of the anterior crossbite and a corrected midline. Conclusions: All three subjects that complied with a non-fixed, DNA appliance/Invisalign protocol showed functional and cosmetic improvements in occlusion without any interproximal reduction.

Acute Myocardial Infarction Associated with Thyrotoxicosis

Thyrotoxicosis is the endocrine disorder which increases oxygen demand and at the same time can induce coronary vasospasm leading to acute myocardial infarction. Patients with thyrotoxicosis induced acute myocardial infarction are rare. The exact mechanism is still not known. The patients with painless thyroiditis shows thyrotoxic phase during early phase which is main culprit for changes in cardiovascular hemodynamic and acute myocardial infarction attack. Here, we present a 23 year old male patient without any risk factors of cardiovascular disease presenting with acute myocardial infarction induced by thyrotoxicosis due to painless thyroiditis.
